General Instructions

1. All work shall be done in accordance with the Standard Specification for Public Works Construction, latest edition and addendums, unless other-wise specified.


2. Traffic Controls throughout permit construction shall conform to the current State of California Manual of Traffic Controls for Construction and Maintenance of Work Zones, latest edition. Arrow board is required.


3. Provisions shall be made for lighted barricades; delineators and traffic control personnel to adequately protect the traveling public during construction and excavation operations. When necessary for public safety, traffic control shall be on duty SEVENTY-TWO (72) hours of each day.


4. A copy of this permit shall be kept at the site of work at all times and produced upon demand.


5. All work authorized under this permit MUST BE COMPLETED WITHIN THE TIME SPECIFIED THEREIN, UNLESS SO COMPLETED, THIS PERMIT SHALL BE VOID. An extention of time may be granted if applied for twenty-four (24) hours before permit expires.


6. The City of Hawaiian Gardens Engineering Division SHALL BE NOTIFIED AT LEAST SEVENTY-TWO (72) HOURS BEFORE START OF WORK BY telephoning (562) 420-2541. Should the inspector fiind work in progess prior to notification by the Permittee and permit is not on-site during construction; work may be stopped for a period of not less than the remainder of the day.


7. An additional inspection fee not less than the double the original fee, will be charged for failure to apply for a permit prior to commencement of work.


8. The holder of any permit and any agent, servant, or employee working for said permit holder on any excavation and fill, shall inform himself and obtain all necessary information as to the existence and locatino of all underground pipes, lines, manholes, wires, signal devices, substructures, and appurtenances of any utility, and the City shall be protected by the Permittee against any damage by reason of any excavation or fill. Any damage caused to such underground installations, appurtenances, or substructures shall be paid for by Permittee. Such repairs as are required shall be made or be caused to be made by the City of Hawaiian Gardens and billed to such Permittee who shall pay the same upon receipt or a statement of the cost of such repair.


9. Form Inspection will be required prior to placement of concrete and at the completion of work. At least seventy-two (72) hours' notice will be required before inspection can be provided.


10. Work shall be performed between the hours' of 7:00 a.m. and 4:00 p.m.


11. Specific Provisions and conditions may be appended to each permit.


12. Dumpsters must have lighted barricades at each end and must be removed after seven (7) days.


13. Comply with additional attached conditions.

  • All oversize loads entering the City of Hawaiian Gardens must conform to the requirements set by the California State Department of Transportation (Caltrans).

 

  • In general, any vehicle over 10'-0" in width, 14'-8" in height, and purple weight class should obtain an oversize load permit from City of Hawaiian Gardens Community Development Department. The purpose is to direct the vehicle to the most efficient route through the City of Hawaiian Gardens to avoid damage to City facilities, unnecessary delay to traffic, and disruption to residential neighborhoods. Pilot Car(s) shall be provided as required by Caltrans, determined during their permit process. If blocking 2 lanes of traffic during peak hours a Hawaiian Gardens Sheriff Department (HGSD) escort may be needed. The HGSD charge for their services separately.

 

  • The Permit issued by the Department of Transportation shall be required to authorize the operation or movement of a vehicle, combination of vehicles, or mobile equipment described. The permit shall not authorize the movement of hazardous materials or hazardous wastes, as those terms are defined by local, state, and federal law.

 

  • When height of load exceeds legal limit, it becomes the responsibility of the permittee to check all underpassed, overcrossings, bridges, overhead wires or other structures for impaired vertical clearance or request alternate routing before commencing travel.

 

  • All loads/vehicles over 8'-6" in width, 80 feet in length or more than 10 feet of overhang shall display warning signs. Signs shall be posted on the front and rear of the vehicle or on the front of the lead vehicle and the rear of the back trailer with multi-vehicle combinations. Sign shall have black letters on a yellow background and comply with Section 502.9 of the Departments "Transportation Permit Manual". Flags shall comply with Section 25104 of the CVC.

 

  • By applying and signing for this permit, the applicant agrees to be responsible for all injuries to persons and for all damage to real or personal property of the City and others directly caused by or resulting from the operation of the applicant's vehicles or combination of vehicles under the conditions of the permit. The applicant shall agree to hold harmless and indemnify of the state and all its agents for all costs or claims arising out of or caused by the movement of vehicles or combination of vehicles under the conditions of the permit.

 

  • The applicant shall provide proof of financial responsibility that covers the movement of the shipment as described. The insurance shall meet the minimum requirements established by law.

 

  • Accidents occurring while operating under permit and requiring a report to the Department of motor Vehicles under CVC Section 16000 shall also be reported to the City by phone and mail.
  • The applicant agrees to carry a copy of the permit in the vehicle at all times and furnish the copy upon request.

 

  • No verbal permits are allowed. Applicants are advised that 48-hours advance notice is required to obtain an oversize load permit. A 2-section manufactured home requires 2 permits, one for each truck load.
