RECREATIONAL BATHING BASIC LICENSE INFORMATION:

Applications for a Public Swimming Pool or Spa are due on June 1st of each year.  The fee is $225.00 per pool and $125.00 per spa.  As per N.J.A.C. 5:23 - 2.18C and 2.20(d), ALL pools must receive an ANNUAL electrical inspection and must maintain a Bonding and Grounding Certificate.  Please contact the Electrical Sub-Code Official in the Building Department for more information on these issues.

  • Seasonal pools must complete the checklist 21 days before opening.
  • Year-Round pools must complete the checklist 30 days before expiration of current license.
  • No Seasonal Pool may open and/or operate until a Pre-Operational Inspection has been performed by The Health Dept.
  • A CB-20 Certification Form & a Bonding/Grounding Certificate must be submitted along with the Operational Checklist.
  • You must contact us 2 WEEKS IN ADVANCE To schedule an inspection.
  • Pool water must be tested by a Certified Laboratory prior to opening.

The Federal Virginia Graeme Baker Pool and Spa Safety Act went into effect on December 19, 2008. All required upgrades and changes to conform to the Act must be made prior to your opening for this coming season.  Specific questions regarding compliance with the Act should be directed to the Consumer Product Safety Commission (CPSC): 800-638-2772 or 301-504-7912.

ONLINE INSTRUCTIONS:

  • Start by selecting 1 Pool/Spa in the dropdown menu even if you have more than 1. You will need to add them later in the application process.
  • Enter the Business Owner information followed by the Facility, Pool/Spa & Supervisor information & click next.
  • Enter the Trained Pool Operation information (if it’s the same as business owner click that button to auto-fill).
  • The Contact Information is whoever oversees the application process.
  • Add Lifeguard information if necessary and click next.
  • This will bring you to the Attachments tab. PLEASE READ THAT PAGE. It has all the information on what is required. YOU MUST UPLOAD ALL YOUR DOCUMENTS IN ORDER TO SUBMIT. Failure to upload all required documents may result in the denial of your application. Click next.
  • This page will show you what the fee is for that pool/spa. You cannot pay online at this time.
  • Type your name in the certification box and click submit. If a red box appears on the left of your screen, you must make those corrections before you can proceed.
  • Once submitted, the Health Dept. will review and notify you via email of the next steps.

TO ADD ANOTHER POOL/SPA you will need the following information:  Applicant Name & Reference # from your prior application.

  • Use the online link and using the dropdown menu click Multiple Pool/Spas. A box will pop up for you to enter the information referenced above & click find my application.
  • All the prior information except the actual pool/spa information will have carried over to this application. Proceed just as you did the prior application. You need to do this for each pool/spa you have.

  • Certification for the replacement of Main Drain Covers in Pool/Spa.
  • Trained Pool Operator Certificate.
  • Middletown Township Electrical Inspection.
  • Life Guard Certificates.
  • A CB-20 Certification Form & a Bonding/Grounding Certificate must be submitted along with the Operational Checklist.
