Traffic Control Plan, Insurance Certificate, Check/Money Order, Bond, Municipal permits, and/or Planning Board Approval. Copies of all local building or any other applicable permits must be attached.   • All work must conform to construction standards of jurisdictions and the requirements and conditions of '{[CNAME]}' Ordinance #20-25 dated 10-07-2020 as amended. The permittee agrees to comply with the current laws of the Federal Government and the Sate of New Jersey. The acceptance of this permit shall be deemed an agreement to abide by all terms conditions and requirements as set forth by '{[CNAME]}'. Notice must be given 48 hours before any permanent paving to the District Road Inspector for inspections.

  • APPLICATION PERMIT FEES AND CASH REPAIR DEPOSIT SHALL BE CALCULATED AFTER REVIEW OF APPLICATION. FEES SHALL BE PAID SEPARATELY; CHECKS SHALL BE MADE PAYABLE TO THE “TOWNSHIP OF MIDDLETOWN.

  • NOTE: UPON ISSUANCE OF PERMIT, THE APPLICANT / CONTRACTOR MUST NOTIFY THE DEPARTMENT OF PUBLIC WORKS & ENGINEERING AT 732-615-2000 Ext (2214) PRIOR TO COMMENCEMENT OF INITIAL WORK AND PRIOR TO FINAL RESTORATION.

  • This application must be made and approved before any work is started, except in case of an emergency. Notice must be provided by telephone immediately following an emergency and followed by a written permit application within 24 hours of notice and/or the next business day. Failure to comply with the emergency procedures may be subject to fines in accordance with Ordinance #2019-3267 dated 10/07/2020. The original permit must be available at the site at all times when work is in progress.
