To complete this application you must show proof of residency. Proof of residency may be attached in the window below. Attach a valid drivers license or deed / mortgage papers that show your name and address in North Haledon. For minors, parents must show same as above, sign for said minor and take responsibility for same.

Code numbers will be changed each calendar year and may be changed during the year if necessary.

Fee Schedule

You will be assigned a code number for access to the tennis courts. This code number should not be given to anyone.
