The undersigned property owner agrees to have the above specified water supply system constructed by a licensed New Jersey well driller in accordance with the provisions of all applicable state and local ordinances.

The undersigned further agrees that he will pay, in addition to the permit and pump test observation fees, all costs directly to a certified laboratory approved by the Administrative Authority, for those water tests as may be required by state and local ordinances.

When the two-part pump test is necessary, the undersigned hereby recognizes that satisfactory test results must be certified by the licensed well driller on forms provided by the Montgomery Township Health Department.
